We attend medical, needs assessment, and care planning appointments on your behalf or alongside your loved one.
We ensure clear communication, advocate for appropriate outcomes, and keep you fully informed — especially important if you are based overseas or unable to attend.

Rest Home Placement

We research, shortlist, and personally assess suitable rest homes and care facilities based on care needs, lifestyle choices, location, availability, and budget.
We coordinate visits (virtual or in-person), provide honest feedback, and guide you through selection and admission.

Aged Care Subsidy & Documentation Support

Navigating Work and Income (WINZ) Residential Care Subsidy, admission agreements, and legal documentation can be complex.
We guide you step-by-step, ensuring everything is completed accurately and efficiently, avoiding delays or costly mistakes.

Move Management & Transition Coordination

We manage the entire move into care — from planning and packing through to setting up the new room so it feels familiar and ready.
Our focus is a smooth, low-stress transition for both the individual and their family.

Property & Belongings Management

We coordinate everything required to manage a home and possessions, including:

Sorting, packing, and distribution of belongings

From a 20 year career in Transport, Logistics and Manufacturing Management, followed by joining his wife Rebecca to run New Zealand's most award winning Yoga and Pilates business, Jeremy has the ability to get things done, while maintaining integrity, empathy and compassion. Logistics, management and a client first mentality mean Jeremy is the ideal person to assist you or your loved one when they need support. Senior Transitions began after we were involved in transitioning multiple family members from independent living, to Residential Care. During the process we noted the challenges faced when someone has to do this alone, and many conversations reinforced the idea that there needs to be some independent assistance available for those who don't have immediate support.
