At Senior Transitions NZ, we provide a complete, end-to-end service supporting older people and their families through the transition from independent living to retirement or residential care in New Zealand.

We specialise in working with adult children who live overseas, out of town, or simply don’t have the time or capacity to manage the process themselves. When an ageing parent can no longer live safely at home, the decisions, urgency, and logistics can quickly become overwhelming. That’s where we step in.

Acting as your trusted local representative, advocate, and project manager, we manage every stage of the journey — from initial assessments and medical appointments, through to rest home selection, subsidy guidance, documentation, relocation, and property coordination.

We take the time to understand your family’s situation and your loved one’s preferences, ensuring decisions are made with clarity, dignity, and care. Whether support is needed urgently following a health event, or you are planning ahead, our role is to reduce stress, save time, and ensure nothing is overlooked.

For families who can’t be here in person — or can’t be everywhere at once — we become your eyes, ears, and hands on the ground, providing regular communication and complete peace of mind.

I wish there was an option for 10 stars. I actually don't know what we would have done if we didn't find Jeremy from Senior Transitions. The road ahead when we landed back in NZ to help our Dad after his stroke was so daunting. Jeremy made this such an easy process, such a nice person, nothing was a problem, he helped us with a timeline and executed on each step. I actually can't believe we got his care, house, belongings and everything in between sorted in the limited time we had in Christchurch. Our dad is in a great home and our stress levels are now very minimal and we can enjoy our time with him

Service Description
While many families choose our full end-to-end service, we also offer individual support services depending on your needs:

Your Eyes, Ears, and Advocate

We attend medical, needs assessment, and care planning appointments on your behalf or alongside your loved one.
We ensure clear communication, advocate for appropriate outcomes, and keep you fully informed — especially important if you are based overseas or unable to attend.

Rest Home & Retirement Living Placement

We research, shortlist, and personally assess suitable rest homes and care facilities based on care needs, location, availability, and budget.
We coordinate visits (virtual or in-person), provide honest feedback, and guide you through selection and admission.

Aged Care Subsidy & Documentation Support

Navigating Work and Income (WINZ) Residential Care Subsidy, admission agreements, and legal documentation can be complex.
We guide you step-by-step, ensuring everything is completed accurately and efficiently, avoiding delays or costly mistakes.

Move Management & Transition Coordination

We manage the entire move into care — from planning and packing through to setting up the new room so it feels familiar and ready.
Our focus is a smooth, low-stress transition for both the individual and their family.

Property & Belongings Management

We coordinate everything required to manage a home and possessions, including:

Sorting, packing, and distribution of belongings
Storage, donations, or sale of items
Cleaning, gardening, and property preparation
Coordination with real estate professionals if the home is to be sold
Flexible or End-to-End Support

You may need help with just one part of the process — or you may want us to manage everything.
We tailor our service to suit your situation, providing as much (or as little) involvement as required.

Meeting the needs of older people
Meeting the needs of older people
From a 20 year career in Transport, Logistics and Manufacturing Management, followed by joining his wife Rebecca to run New Zealand's most award winning Yoga and Pilates business, Jeremy has the ability to get things done, while maintaining integrity, empathy and compassion. Logistics, management and a client first mentality mean Jeremy is the ideal person to assist you or your loved one when they need support. Senior Transitions began after we were involved in transitioning multiple family members from independent living, to Residential Care. During the process we noted the challenges faced when someone has to do this alone, and many conversations reinforced the idea that there needs to be some independent assistance available for those who don't have immediate support.

ChCh Rest Homes: Why Expensive Isn't Always Best

Finding the perfect Canterbury aged care facility isn't just about the budget—it's about the lifestyle fit. I recently assisted a 71-year-old client transitioning from Burwood Hospital to a rest home in Sockburn.

While he initially budgeted $70/day for a Premium Accommodation Charge (PAC), our discovery process revealed that his true priorities weren't a larger floor plan or a view. Instead, we focused on high-quality meal options, staffing ratios, and mobility scooter storage.

By opting for a 'Standard' room over a Premium Room or a Occupational Rights Agreement (ORA) suite, we found a better lifestyle match. The result? He is thriving in his new community, and he is saving over $25,000 a year in premium fees.

As a Senior Transition Specialist in Christchurch, my goal is to ensure families don't overpay for space they don't need, while securing the care they actually deserve.
