Our personalised support services

-Personal Care: Personal Care Support at My Holistic Care is designed to assist individuals with essential daily living activities while preserving dignity, privacy, independence, and personal choice. Support may include, showering and bathing assistance. Assisting with maintaining personal hygiene, dressing, grooming and continence support.

-Home help services: A clean, safe, and organised home plays an important role in wellbeing. We assist with general household tasks such as vacuuming, mopping, wet- dusting, laundry, changing bed linen, dishwashing, light tidying, and other domestic duties, helping clients remain comfortable and independent in their own homes.

-Respite Care: Caring for a loved one can be rewarding but also demanding. Our respite services provide family members and primary carers with valuable time to rest, attend appointments, work, or recharge while knowing their loved one is receiving compassionate, reliable care.

-Cognitive Support: Our team provides patient and compassionate support for individuals experiencing memory loss, dementia, or cognitive impairment. We assist with maintaining daily routines, orientation, meaningful engagement, supervision, companionship, and promoting a familiar, reassuring environment.
Our trained support workers communicate patiently and respectfully, adapting their approach as needs change over time.
This service does not include diagnosis, medical treatment, or specialised therapy. All support is delivered within scope, aligned with agreed care plans, and in accordance with organisational policies and NZS 8134:2021 standards.

-Disability Care: We provide personalised support to people living with physical, intellectual, neurological, or sensory disabilities. Our services are designed around individual goals, encouraging independence, participation in daily activities, confidence, and inclusion within the community.

-Shopping and Transport support: We support with running errands and grocery shopping. Support to attend social of community activities. To encourage and maintain social connection and community engagement.

-Meal prep and Tech support: Good nutrition is essential for maintaining health and wellbeing. We can assist with meal planning, preparing meals, special dietary requirements, hydration reminders, and maintaining a tidy kitchen, ensuring clients enjoy healthy, balanced meals.
We also help clients stay connected, independent, and confident when using everyday technology. Our support workers can assist with smartphones, tablets, computers, video calling, online appointments, emails, online shopping, accessing health portals, and using apps for communication and wellbeing. We provide patient, one-on-one assistance at a pace that suits each individual, helping clients feel comfortable and connected with family, friends, and essential services.

-Companionship Support: Meaningful social connection improves quality of life. Our compassionate support workers provide friendly companionship through conversation, shared activities, hobbies, walks, games, reading, and emotional support, helping reduce loneliness and promote overall wellbeing.

- Health and Wellbeing Support: Provides non-medical assistance to promote overall wellbeing and support individuals following illness, injury, or hospital discharge. This service complements, but does not replace, medical or nursing care. Support focuses on comfort, recovery, routine, and maintaining wellbeing at home. Services include:
- Medication prompting or reminders.
- Nonclinical assistance with simple wound care routine.
- Monitoring of skin and general wellbeing.
- Support with prescribed daily routines and recovery plans
- Post- hospital discharge and wellbeing support and coordination with health professional where appropriate.

My Holistic Care works in partnership with individuals, families, and whānau to plan, deliver, and review support services. Our flexible service model allows support arrangements to adapt over time as needs and circumstances change, supporting people to remain connected to their homes, communities, and way of life.

Led by experienced healthcare professionals and supported by Registered Nurse clinical leadership, we work collaboratively with clients, families, GPs, allied health professionals, hospitals, and community organisations to deliver high-quality support that promotes independence, wellbeing and peace of mind.

We specialise in supporting people living in rural and semi-rural Auckland, where access to consistent and reliable support services may be limited. By prioritising locally based support workers and continuity of service, we aim to provide dependable, responsive, and culturally respectful support that reflects the realities of rural living.
