Meeting the needs of older people
| My Holistic Care works in partnership with individuals, families, and whānau to plan, deliver, and review support services. Our flexible service model allows support arrangements to adapt over time as needs and circumstances change, supporting people to remain connected to their homes, communities, and way of life. Led by experienced healthcare professionals and supported by Registered Nurse clinical leadership, we work collaboratively with clients, families, GPs, allied health professionals, hospitals, and community organisations to deliver high-quality support that promotes independence, wellbeing and peace of mind. |
