CareWay provides practical, calm support during the high-risk gap after discharge - when family can't
be there and the person needs more than just a ride.

WHO THIS IS FOR
• Older adults living alone
• Early discharges (same-day procedures)
• Mobility issues, fatigue, frailty, dementia/confusion
• Patients needing pharmacy and follow-up support
• Anyone who struggles with waiting rooms, forms, and settling back at home

WHAT I DO (SUPPORT + TRANSPORT)
Door-through-door, stay-with-you support includes:
• Safe pick-up and escort inside (not curb drop-off)
• Waiting support at appointments (clinic / hospital / specialist)
• Pharmacy pick-ups, prescriptions, and essential errands
• Help collecting discharge paperwork and instructions
• Return home and settle in (safe handover, comfort check)
• Family updates (text / phone)
• Can include: groceries, urgent supplies, documents

Transport is included as part of the support.
MOST COMMON REQUESTS
1) Home Safe Discharge Support - pick-up -> wait -> pharmacy -> home -> settle-in
2) Appointment Support - door-through-door escort + waiting + safe return
3) 72-Hour Gap Support (Short term) - daily check-in support, essential errands, pharmacy runs
