Service Description
Our Wesley Community Action's Older Persons Team works in partnership with older people to support them to maintain their independence, avoid early or inappropriate admission into residential care, and achieve a quality of life which reflects their own priorities and goals.

Our team of professionals uses a holistic approach to achieve this. We provide case-management of the various services involved in a person's life, along with advocacy, home visits, outings, transport to essential appointments plus facilitation of access to family and community resources according to the older person's wishes.

The Wesley CA Older Persons Team provides, under contract to the Hutt Valley and Capital and Coast DHBs, the following services:

Community support services for older people with complex needs including age related disability support needs, supporting them to live independently in the community.

A key outcome of the services is to prevent the need for premature admission into a residential care service.

The target groups are people aged 65 and over with complex physical needs and a high social need who might otherwise require inappropriate or premature placement into residential care because their support needs cannot be met by current services.

Referrals are made to the local Care or Service Coordination Centre who carry out a needs assessment and if appropriate, request services from the Wesley Older Persons Community Team.

The aim of the Wesley Older Persons service is to assist the older person to regain social skills and motivation and to link them back into their local community through a process of individual service planning. The community social worker assists the person to identify and then work toward achieving their specific goals within this context.

Services provided will vary depending on the need but may include things such as:
Ø assisting and where necessary advocating with health, housing, financial and environmental issues; preparatory work to enable home support services to be provided; goal setting;linking to community groups and services; facilitating meaningful activity and capacity to contribute where desired

Ø assisting with the transition to residential care when the safety of the person becomes a major concern in the absence of other social support networks.
