Service Description
SERVICES AVAILABLE:

* Personal/Attendant care
* Assistance with showering/bathing & dressing
* Toileting

HOW TO APPLY?

You or any other authorised person can request subsidised home support via your General Practitioner (GP).
However, if you are an existing Home Support client and receive regular weekly services, then we can refer you on to many other agencies or health care providers such as:
NASC to have your service hours reassessed; OT for equipment and modification requirements; Alzheimers Northland for support and whanau training; our mobile nursing teams for monitoring and support within your own homes, and many more. Phone us direct if you are unsure about how we can help you.

* ALL New client referrals are sent by your GP direct to NASC at Whangarei Hospital, for triage to local NASC teams.
* Whanau assistance and participation is encouraged.

WHAT HAPPENS NEXT?

The NASC (Needs Assessment Service Co-ordinator) will visit you in your home and if they determine you need home support they will authorise care with an agency of your choice. The client has the choice of three home support agencies providing care in the Far North; Te Hiku Hauora is the largest Health Provider in this area, thereby offering a wide range of health services, including Home Based Support Services to the entire Far North community. The Needs Assessor will offer you information on each of the three providers, and when you have made your decision she will refer to the agency of your choice.

Upon receipt of the referral, our Co-ordinator will phone you to make an appointment to visit you at home and arrange for you to meet with your allocated support worker. In some instances and at the discretion of the Manager, your preferred support worker may commence the process to gain employment with our organization, provided they meet the current employment criteria of attaining the industry minimum of Careerforce Level 2 Foundation Skills national certificate.

SUPPORT WORKERS

We employ Support Workers in all areas and are responsible for the legal requirements of their employment.

We provide Support Workers with orientation and on going training.

If your preferred Support Worker is not suitable, you always have the choice of another Support Worker from our pool.

WHO QUALIFIES FOR FREE SERVICE?

Any person with a physical, intellectual or sensory disability who holds a current Community Services Card meets the criteria for Household Management supports, provided there is no other capable adult/whanau living in the home.

Personal care (PC) support is assessed on the individuals requirements. Short term care PC is sometimes allocated on discharge from hospital.
Long term PC is usually allocated to assist clients maintain their personal regime on a daily basis.

HOUSEHOLD MANAGEMENT
Be unable to perform normal daily household tasks which allow you to live independently, e.g. laundry, vacuuming, dusting, bedmaking, meal preparation & shopping.


PERSONAL CARE
Any person requiring assistance to perform normal personal care activities for daily living eg: bathing, personal hygiene, dressing & undressing, personal grooming, mobility and social interaction.
