Description
Our Home Care service aims to support you to retain your independence while living safely in familiar surroundings. Our model of care is person-centred, and involves preparation of an individual support-plan that is developed in partnership with you and your natural supports. We focus on your individual strengths and abilities, providing support where needed and take an innovative approach to delivering the best possible health and community care services.
Our service is available to anyone living at home who is recovering from and illness, accident or surgery and also for those with a life-limiting illness or who need long term support.
Our services are provided with a restorative focus. A Registered Nurse will work with you and your whanau to establish your health goals in order to enhance or maintain your independence. They will continue to meet with you regularly to review how the service is meeting your needs and adjust the service plan as required. The day to day care and support is provided by skilled and experienced Support Workers with the oversight of a Registered Nurse.
What services are available?
We can support you with
• Housekeeping, cleaning and maintain your household
• Personal care to assist you to look and feel your best
• Grocery shopping either with you or on your behalf
• Medication management assistance
• Meal preparation
• Managing chronic conditions
Care after an injury
Nurse Maude provides a serious injury service that is overseen by two Registered Nurses. This service is generally for people who have a spinal cord or brain injury and need a more intensive package of care. The Registered Nurse leads the healthcare team, manages you care package, addresses and queries you may have and liaises with ACC if required and arranges any additional supports you may need. We also provide homecare services to those returning to independence after a minor injury.
We work in partnership with Access Home health for ACC Personal Care and Household Assistance or the Serious Injury Service so ask your doctor or hospital staff put Access on the referral form and when Access contacts you to arrange your care let them know you would like Nurse Maude to provide that care.
CREST
The Community Rehabilitation & Enablement Support Team (CREST) provides an intensive programme of two to six weeks home-based assessment and rehabilitation support by a trained home support worker guided by nursing services and allied health. The service provides and promotes rehabilitation of clients in their home, working in collaboration with Older Persons Health Specialist Services and in communication with the clients’ own GP Team and community pharmacist.
Who can receive services?
People of all ages receive support from Nurse Maude. Your situation will determine what support you are eligible for. Most referrals are made by the GP practice or our public hospitals. We also receive referrals from needs assessment and service coordination agencies, ACC and private hospitals. Just let them know you want Nurse Maude to provide your care.
Homecare services are free to those assessed as meeting eligibility for services. There is also a private payment option for those not eligible for funding.

Service Description
| Additional services provided by Nurse Maude
District nursing services The district nursing service provides district nursing to the city and outlying rural areas of Christchurch seven days a week, 24 hours a day.This includes wound care, palliative care, medications, catheter care and intravenous therapy. Diabetes The diabetes service provides support, advice and education to General Practice to support people with diabetes. Wound care clinic The wound care clinic runs outpatient clinics at Nurse Maude Merivale to assist with managing complex woulnds. Continence advisors The continence advisors run outpatient clinics within Nurse Maude in Merivale as well as outlying areas such as Kaiapoi. Stomal therapists The stomal therapist work in the community and visit patients in the both public and private surgical hospitals within Christchurch pre and post surgery. Intraveneous Therapy service The intravenous therapy service is run in the community and delivered by Nurses in the IV clinic based at Merivale as well as by district nurses in the community. This enable people to receive their intravenous therapy in a community setting rather than in a hospital. Palliative care Nurse Maude’s Hospice Palliative Care service is provided across the greater Canterbury region, including Kaikoura, Ashburton, Banks Peninsula, Hanmer Springs and across the West Coast. The Hospice Palliative Care Service has a multi-disciplinary team which includes trained nurses, doctors, a Paediatric Palliative Care Clinical Nurse Specialist, allied health professionals and a Family Support team which includes a kaiāwhina and pastoral care worker. Hospital Nurse Maude Hospital is a 40-bed, fully licensed hospital with registered nursing and medical staff committed to the philosophy that the welfare of the patient must come first.The hospital specialises in aged care and those needing palliative care.
