Service Description
SERVICES AVAILABLE:

* Shopping
* GP/Specialist appointments
* Personal/Attendant care
* Assistance with showering/bathing
* Personal Hygiene

Carer Support may be allocated to assist full time carers to take a break

HOW TO APPLY?

You or any other person can apply for care on your behalf.

* We can help you arrange for a needs assessment and a co-ordinator visit.
* A trained needs Assessor will contact you and assess you for any supports you need.
* Whanau assistance and participation is encouraged.

WHAT HAPPENS NEXT?

The Needs Assessor will visit you in your home and if they determine you need home support they will authorise care with an agency of your choice. The client has the choice of three home support agencies providing care in the Far North; Te Hauora O Te Hiku O Te Ika is the Maori Provider in this area.The Needs Assessor will offer you information on each of the three providers, and when you have made your decision she will refer to the agency of your choice

Our Co-ordinator will phone you to make an appointment to visit you at home and arrange for you to meet with your allocated support worker. In some instances and at the discretion of the Manager, your preferred support worker may commence the process to gain employment with our organization.

SUPPORT WORKERS

We employ Support Workers in all areas and are responsible for the legal requirements of their employment.

We provide Support Workers with orientation and on going training.

If your preferred Support Worker is not suitable, you always have the choice of another Support Worker from our pool.

WHO QUALIFIES FOR FREE SERVICE?

Any person with a physical, intellectual or sensory disability who holds a current Community Services Card.

Have no other capable adult/whanau living in the home.

HOME SUPPORT
Be unable to perform normal daily household tasks which allow you to live independently, e.g. laundry, vacuuming, dusting, bedmaking, meal preparation & shopping.


PERSONAL CARE
Any person requiring assistance to perform normal personal care activities for daily living eg: bathing, personal hygiene, dressing & undressing, personal grooming, mobility and social interaction.
