Service Description
Some things make for a healthier, happy life no matter what your age

Restorative Home Support is a quality home care service offering household help and personal care for older people and those with disabilities, either temporary or ongoing.
· Flexible care and support to assist individuals to live independently in their own homes.
· Personal care and household management
· Improve health and wellbeing
· Assist clients regain and maintain independence
· Support the household
· Promote social contact with others
· Improve mobility and quality of life

Community Social Work: Support for people over 65 in the community and their
families. If you are concerned about any health, emotional or social issues, contact the
Social Worker.

Falls Prevention: Tai Chi for people over 65 years who have problems with strength
and balance and may have pain or other disabilities. Tai Chi exercise benefits physical stability and balance to help prevent falls. Classes are held in a number of locations around the region. Contact the Falls Prevention Co-ordinator.

Totara Nelson: An activity programme from 10am to 3pm where older people can
get together and enjoy good company, friendship, activities and mutual support. Meals are included. Transport is possible and you may be eligible for financial assistance. Contact the Co-ordinator.

Harakeke Nelson: A day activity programme in partnership with Alzheimers Society
for people with memory loss, eg. Alzheimers, stroke and other related dementias. 10am to 3pm at 360 Annesbrook Drive. Transport and meals are included. Financial support is usually available. Contact the Day Services Manager.

Subsidy Details
Subsidies are available for household help and personal cares through contracts with ACC and Health Funding Authority. We can also access Disability Support subsidy from Work and Income. (If you are eligible.) N.B. Meal subsidy applies to ACC clients
