Our fully funded services are available via referral from your doctor, local needs assessment agency or through ACC. For Maori clients we have a comprehensive Maori Health policy in place and are able to supply Maori Care Providers if required. |

| HOW CAN YOU ACCESS OUR SERVICES? Many people can access our services, some services may be subsidised, some fully paid, or you may choose private care. Referrals can be received from: * Yourself for private or subsidised clients * Family/Whanau * Friend * GP * Assessment unit * Disability assessors * Care givers for carer support * Hospitals Following a referral, a needs assessment will be completed by the appropriate assessment services to help determine what care you require, and to what extent it will be subsidised. |
